The 5th why would be your root cause. You use the 5 why analysis so that you can drive to get your root cause of the problem. Once you then have the true root cause your employees will then be able to determine the appropriate countermeasure for this particular situation.

Is the fifth why the countermeasure, or is the countermeasure still based on the over all problem? Or perhaps the comments begin at “5 Whys:’?
